Application Submission

1.	Read and study carefully all materials attached to this call for applications.
2.	Select 3 qualified persons to complete a Confidential Evaluation/Recommendation form for you. Then distribute to each person a form with a pre-addressed, postage-paid envelope for them to return the form directly to NPC in. Additional instructions that you should become aware of are on the form. Four copies of the form are enclosed for your use. If necessary, you may Xerox more copies. Encourage all whom you select to be prompt in completing and mailing the form!
3.	Complete the enclosed application form. Be sure to sign.
4.	Submit with application a one page statement which answers the question "Why Do I Want to Become a Cosmetologist, Nail Technician or Instructor?" (whichever program you are applying for.)
5.	Submit with application a maximum one page statement which gives "My General Background Information" i.e. (a condensed resume).
6.	Submit one of the following with your application or bring two copies to the orientation/testing session (see item 9). A copy of your high school diploma or G.E.D. certificate. Or official transcript from your high school showing completion of at least ten credits which MUST include two English, one math and one science. If none above are applicable, you must present a picture I.D. with proof of age showing you are twenty-three years old or older.

7.	Return completed application materials and deposit check, to: Secretary to Division of Math and Science (Attention Cosmetology) Northland Pioneer College P.O. Box 610 Holbrook, AZ 86025 Note: Please return application form soon as possible. If there are more qualified serious applicants than there are available spaces in the program, selection criteria may involve assessed competency levels and dates of properly completed applications (including letters of recommendation). Early application is strongly encouraged.
8.	Attend general aptitude/dexterity test session and program orientation meeting as follows: Date of test will be announced each semester, (check with campus coordinator) Time: Start 9:00 a.m. (Test time plus program orientation/interview time = 4-6 hours total, a sack lunch may be brought.) Place: NPC Show Low and Winslow Campuses PLEASE REMEMBER- this is an interview, dress accordingly. A Financial aid advisor will be available to meet with you on this testing date.
